OWNER HEALTH CERTIFICATION & LIABILITY WAIVER

For Boarding, Day Camp, Dog Park Use, Grooming or Training at Kennel Resorts

I,_______________________________________________________________________________________, hereby certify that my dog(s)and/or cat(s):

_________________________________________________________________________________________(plus any additional dog(s)and/or cat(s) that I may present in the future) will be in good health and will not be ill with any communicable condition when they come to Kennel Resorts (KR) for boarding, day camp, grooming, training or to use the dog parks within 30 days. I have read and understand the following:

OWNER ACKNOWLEDGES AND UNDERSTANDS THAT THE EMPLOYEES OF KENNEL RESORTS ARE NOT VETERINARIANS AND ARE NOT EXPECTED TO DIAGNOSE OR DETECT ILLNESSES IN THE GUESTS THAT ARE STAYING AT KENNEL RESORTS. IN ADDITION, OWNER UNDERSTANDS AND IS AWARE THAT VACCINES DO NOT PROTECT AGAINST ALL COMMUNICABLE ILLNESSES THAT MAY AFFECT A GUEST. Kennel Resorts strives to provide you with suitable services as contemplated in this Agreement. Customer understands the risk of hiring Kennel Resorts to provide these services, and fully indemnifies and holds harmless Kennel Resorts for all accidents, injuries, and harm otherwise caused to the pet or any third party in conjunction with these services. All parties acknowledge that animals sometimes act outside of a caregiver’s control, and that accidents do happen. Accordingly, Company shall under no circumstances be responsible for incidents that occur with relation to the pet during the provision of care, including but not limited to: injuries to the pet, caregiver, or other third party; or the sickness, death, or disappearance of a pet. Your execution of this Agreement indicates your acceptance with these terms.

______1. Dogs/Cats are animals and by nature can be unpredictable and inherently dangerous to themselves and other animals and persons during participation in the Activities. I understand that certain other risks to my pet are inherently dangerous during participation in the Activities, and may include, but are not limited to, ingesting objects or substances, physical injuries, bloat, bee or insect stings, or contracting illnesses/infections (kennel cough, worms, fleas, etc.).

______2. I understand that I am solely responsible for any harm caused by my dog(s) and/or cat(s) while attending Kennel Resorts and, I assume all risk of damages by my pet to other pets, Kennel Resorts’ employees, agents or any kennel asset and Kennel Resorts shall have no liability for any harm to such pet. I shall defend, indemnify, and hold harmless KR parties against any and all losses, damages, liabilities, claims, actions, judgments, settlements, interest, awards, penalties, fines, costs, or expenses of whatever kind, including veterinary bills and reasonable attorney’s fees, incurred by KR party arising out of or resulting from any claim related to my dog(s)/cat(s) participation in boarding, exercising, grooming, and training.

______3. I further understand and agree that in admitting my pet(s) to Kennel Resorts that Kennel Resorts has relied on my representation that my pet(s) is/are in good health free of any communicable illness for the last 30 days, and has/have not harmed or shown “serious” aggressive or threatening behavior toward any person or another pet. Nor is my pet used as a guard/protective/watch dog. All pets are required to have vaccinations up to date through their stay by a licensed veterinarian and I am responsible for providing ongoing verification of current vaccinations.

______4. I further understand that Kennel Resorts and staff will not be liable for any problems that develop, provided that reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever arising from my pet(s) boarding, exercising, grooming, or training at Kennel Resorts. I understand that Kennel Resorts reserves the right to delay notifying me about minor injuries and or illnesses until I pick up my dog. I hereby expressly waive and release any and all claims, now known or unknown, against any KR Parties on account of injury or death or damage arising out of or relating to my dog(s)/cat(s) participation in boarding, day camp, grooming, & training, whether arising out of the negligence of any KR Parties. I covenant not to make or bring any such claim against KR Parties, and forever release and discharge all KR Parties from liability under such claims.

______5. I understand that escape (climbing, digging, and enclosure destruction) is something that can happen and release all liability from Kennel Resorts if my pet escapes. Please inform Kennel Resorts of any known escape behaviors.

______6. I further understand and agree that if my pet requires medical attention for any reason, as determined at the discretion of Kennel Resorts’ staff, I will assume full financial responsibility for that care including transportation costs. I hold Kennel Resorts harmless in the event of an injury or accident during such transportation. I agree and understand that any problems (medical or otherwise) that relate to or develop with my dog(s) will be treated as determined by the KR Parties, in their sole and absolute discretion, and that I assume full responsibility for any and all expenses involved, including veterinary bills.

______7. I understand Kennel Resorts has a 48hr cancelation policy and I will be charged my $25.00 deposit if I do not cancel in that time frame or no show. All cancellation less than 48 hrs will be charged $25.00 by forfeit of deposit or to credit card on file.

______8. Please note our pick up time is NOON and all parties will be charged an additional day of boarding if pick up is after 12:00 p.m.; the only exclusion is Sunday. (pick up hours between 4-6 p.m.) All parties will be charged for first boarding night on day of drop off regardless of dropoff time.

______9. Hours of operation are Monday –Friday 8:00a.m.-6:00p.m., Saturdays 8:00a.m- NOON and Sunday between 4:00-6:00 p.m. (for pick up only). Please check our website for holiday hours. We are closed for all major holidays.

______10. I received a copy of the document SUMMARY OF POTENTIAL ISSUES at Kennel Resorts. It is my responsibility to read and understand the content of that document.

Pea Stone Notice

Kennel Resorts Pea Stone Notice

This Pea Stone Notice is made by Kennel Resorts and/or their affiliates, employees, managers, members, officers, agents, successors, assigns, clients and (collectively, the “KR Parties”). Kennel Resorts uses pea stone in their outdoor play areas at the KR locations. Pea stones are small, round stones without sharp or rough edges. Pea stone is a recommended surface for dog play areas because it is a forgiving surface that is easier on dog’s joints than concrete. Pea stone also allows urine to sink through to the ground below while fecal matter sits on top of it, where it is more easily noticed and picked up as promptly as possible. This allows the KR Parties to keep the outdoor areas generally free of waste so dogs are not stepping in it while playing. At times, a dog who is an avid feces eater will get to the feces before the KR Parties are able to remove it. As such, a dog who eats feces may also ingest pea stone. Pea stone alone is usually harmless and will usually pass on its own (the dog will either vomit it up or it will pass in the feces). But often times they will consume more than they can pass and may require surgery. Many dogs have no issues running and playing on pea stone. Some dogs (especially dogs who have not built up tough paw pads due to their youth or lack of exposure to surfaces other than those in their homes and grassy outdoors) may experience irritation. While walking and running, pea stone can rub in between paw pads, causing redness and irritation. Some dogs may lick their paws to relieve the irritation or may favor an exceptionally sore paw by limping. Please be sure to prevent your dog from licking their paws as this will exacerbate the soreness. To prevent or reduce future irritation from the pea stone, a paw wax or balm can be used, subject to your dog’s veterinarians approval. Paw wax is a substance that is applied to the paw pads and in between the paw pads. It provides a protective barrier which reduces irritation caused by rough surfaces such as pea stone. The paw wax oftentimes contains healing properties as well. Please always consult your veterinarian if you have any concerns about your pet(s).

All Rights Reserved. Kennel Resorts reserves the right to make changes to the Pea Stone Notice without advanced notice and to require client to resign to continue participation in the Activities. This Pea Stone Notice applies to all dogs that enter our property for any reason.

In the event my pet experiences a cardiac, respiratory or other life-threatening emergency that requires resuscitative or other urgent care measures, such as cardiopulmonary resuscitation (CPR), positive pressure ventilation, emergency drugs, or other similar measures, I request that the veterinarian and/or trained staff at Kennel Resorts pursue such medical care as indicated below.
